Job Description

American Traveler is seeking a Physical Therapist for Home Health assignments in the Inland Empire area, requiring at least 1 year of recent skilled PT experience and Oasis proficiency.

Responsibilities

• Home Health setting covering Inland Empire, CA and surrounding regions
• Day shifts with 5x8 hour schedule, typically 8:00–16:30
• 1:1 patient ratio with up to 6 patients per day and 30 productivity points standard
• One weekend shift per contract required
• No floating between units
• No on-call requirement
• Travel pairs are accepted, but each traveler is assigned to their own territory
• Clients are grouped to minimize drive time, though a maximum radius of 50+ miles may be occasionally necessary
• Primary duties include patient evaluation and treatment in a home health environment
• Mileage is reimbursed at the IRS standard rate
• Standard business or casual attire; no specific scrub color required
• Parking is free at this assignment
• Must be comfortable working independently in the field
• First-time travelers will not be considered for this position
• Former permanent employees must have a minimum 6-month separation and HR approval before eligibility as a traveler

Requirements

• Active PT license required (license required if applicable)
• Minimum 1 year of recent, relevant skilled home health PT experience
• BCLS certification required
• Oasis experience required for staff completing Start of Care visits
• Proficiency in HomeCare HomeBase (HCHB) preferred
• Active driver's license and reliable personal transportation required
• Timely communication and documentation skills required
• Completion of a Hospice/Home Health Checklist is mandatory
• RTO must be submitted at time of application
• Must provide 2 professional references from a supervisor within the past 2 years
