Job Description

American Traveler is seeking a Physical Therapist for a home health contract in Sonoma, CA, requiring at least one year of recent skilled PT experience and Oasis familiarity.

Responsibilities

• Home health setting covering Sonoma, CA and surrounding areas
• Day shifts, 5x8 hours per week, with standard hours from 8:00 to 16:30
• Visit up to 6 patients per day with a 1:1 patient ratio and 30 productivity points standard
• One weekend shift required per contract
• No on-call and no floating between units required
• Travelers are considered local if within a 50-mile radius and will be accepted at the same rate
• HomeCare HomeBase (HCHB) is used for charting and EMR documentation
• Own transportation is required, and distance may occasionally exceed 50 miles in a day
• Responsibilities include providing home health physical therapy services and completing Oasis assessments
• Free parking and mileage reimbursement offered per IRS standard
• No uniform color requirement for attire
• Not open to first-time travelers
• Request time off must be discussed with hiring manager
• Patient visits are scheduled efficiently to reduce drive time

Requirements

• Minimum 1 year of recent skilled physical therapy experience required
• Current BCLS required
• Experience with Oasis documentation is required
• Experience with HomeCare HomeBase (HCHB) required
• Must have valid driver’s license and reliable transportation
• Pending license accepted; license required if applicable
