HOME HEALTH REGISTERED NURSE | DETAILS AND COMPENSATION:

• Location: Carmel IN– 100% On-site
• Payrate: $48.69/hr
• Required Availability: Full-Time | Monday–Friday, 8 AM – 5 PM (No weekends required)

HOME HEALTH REGISTERED NURSE | SUMMARY AND HIGHLIGHTS:

The Home Health Registered Nurse will provide direct nursing support and case management, ensuring health maintenance, disease prevention, and high-quality care for individuals in home and community settings.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH REGISTERED NURSE | RESPONSIBILITIES:

• Assess and implement individualized care plans.
• Observe, plan, implement, and evaluate nursing support for individuals with acute or chronic conditions.
• Supervise care plans and daily work assignments of supporting staff.
• Coordinate communication between healthcare professionals to address patient needs.
• Manage medication processes, including ordering, monitoring, and documentation, in compliance with policies.
• Maintain accurate nursing records and progress reports.
• Provide health and safety training for new employees and ongoing recertifications.
• Ensure unlicensed staff are trained on person-specific health needs.
• Support care transitions and collaborate with discharge coordinators as needed.

HOME HEALTH REGISTERED NURSE | REQUIREMENTS :

• Education: High School Diploma or GED (Verifiable).
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Active Indiana RN license or compact state license.
• Home health and OASIS experience strongly preferred.
• Strong organizational and communication skills.
• Ability to work independently and as part of a team.
