The Registered Dietitian in Cardiac Rehab provides comprehensive nutrition assessment, care planning, education, and therapy specifically tailored for cardiac rehabilitation patients. This role involves conducting individual consultations, live cooking classes, and nutrition workshops across multiple sites while ensuring compliance with healthcare regulations. The dietitian collaborates closely with the cardiac rehab team to deliver quality medical nutrition therapy and community education.
Overview The Clinical Dietitian is responsible for the nutrition screening, assessment, monitoring, evaluation, and education of Cardiac rehab patients through an integrated approach to nutrition care. This position will work Monday, Tuesday and every third Wednesday: CATR Cardiac Rehab. Wednesday, Thursday, Friday: Curtis Cardiac Rehab Qualifications Education Level Field of Study Bachelor's Degree Licenses/Certifications Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR) And Registered Dietitian (RD) - Commission on Dietetic Registration (CDR) And Driver's License - Varies Routine weekly travel to other CFH sites is expected. Required to drive your own vehicle to sites if a department vehicle is not available Work Experience Length of Experience Healthcare 1+ years Specialized Knowledge and Skills Requirements Required English Skills Basic reading skills Basic writing skills Basic oral skills Communication Skills Ability to respond appropriately to customer/co-worker Interaction with a wide variety of people Maintain confidential information Ability to communicate only the facts to recipients or to decline to reveal information Ability to project a professional, friendly, helpful demeanor Computer Skills Intermediate computer knowledge: Ability to troubleshoot minor problems within a Windows OS, operates within a network environment, uses spreadsheet, database, word processing, and internet applications proficiently. Learns new applications without difficulty and is able to aid others in immediate work area with computer questions. Responsibilities Essential Functions PROVIDES COMPREHENSIVE NUTRITION CARE OF PATIENTS ACCORDING TO POLICIES & PROCEDURES
Responsible for nutritional assessment completions 100% per policy
Meets charting guidelines 100% of the time determined by chart audits
Provides quality medical nutrition therapy as appropriate on cardiac rehab patients including necessary follows and in accordance with facility policy
ACTIVELY PROVIDES NUTRITION SUPPORT THROUGH ASSESSMENT, DEVELOPMENT OF A NUTRITION CARE PLAN, CONDUCTION OF LIVE COOKING COURSES AND NUTRITION WORKSHOPS
Works with other Cardiac Rehab Team members to provide nutrition therapy to patients in a timely manner.
Writes and manages care plans for all patients to be kept in their files with the department
Performs 1:1 consultations with all newly enrolled cardiac rehab patients as appropriate
Conducts nutrition workshops to cardiac rehab classes as outlined in the education calendar
Performs live cooking courses for cardiac rehab patients on a weekly basis as outlined by the department
PROVIDES NUTRITION CARE OF PATIENTS ACCORDING TO REGULATORY BODIES SUCH AS JCAHO, IDPH, CARF, ETC. AND CURRENT TRENDS AND KNOWLEDGE ACQUIRED ANNUALLY
Maintains knowledge of regulations
Analyzes performance compared to regulations and offers ways to better, efficiently comply to provide optimum nutrition care
Fulfills CDR and IDPR education requirements and maintains report portfolio with education receipts for IDPH or CDR audit
PROVIDES NUTRITION EDUCATION TO PATIENTS, FAMILIES, COMMUNITY EVENTS, STAFF GROUPS, ETC.
Develops and utilizes a variety of processes to determine patient education needs, ie consults, assessments, etc.
Works cooperatively with cardiac rehab team members in determining and providing patients' education needs
Provides nutrition education to patients, family members, staff, and community groups
