Job Description
American Traveler is seeking a Physical Therapist with at least 1 year of home health experience for a full-time day shift assignment.
Responsibilities
- Home health setting providing care to patients in their residences
- Service areas include White River Junction, Lebanon, Bradford, and Windsor across Vermont and New Hampshire
- Day shift schedule
- Average visit length is approximately 45 minutes
- Responsibilities include evaluating patients, developing and implementing treatment plans, and documenting goals and progress
- Educate patients and their families on treatment rationale and procedures
- Coordinate care and communicate effectively with the interdisciplinary healthcare team
- Role offers autonomy with strong team support and a focus on meaningful patient relationships
- No radius restrictions for local candidates
Requirements
- Bachelor’s degree in Physical Therapy required
- Current licensure as a Physical Therapist in VT and NH or an unencumbered compact license required if applicable
- Minimum 1 year of home health PT experience required
- Current BLS/BCLS certification required
- Rehabilitation certifications such as ASHA, CHT, or ACP Training are required if applicable
