Title: RN/Case Manager
Location: Santa Fe, NM
Shift: 8:00 AM – 4:00 PM
Duration: 32 Weeks
Responsibilities:
- Coordinate and manage patient care plans across the continuum of care.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure quality patient outcomes.
- Provide education and support to patients and families regarding discharge and post-hospital care.
- Monitor patient progress and adjust care plans as needed.
- Ensure compliance with hospital policies, procedures, and regulatory requirements.
- Participate in discharge planning and transition-of-care coordination
Requirements:
- Active RN license (New Mexico or Compact State).
- Minimum 3 years of experience as a Registered Nurse Case Management or related specialty.
- Discharge planning experience required.
- Strong communication, organizational, and critical thinking skills.
