JOB SUMMARY:

To assist the transplant coordinators in all activities related to patient care.

The coordinator assistant will communicate closely with the coordinator to provide a continuum of care for the patient.

MAJOR DUTIES / CRITICAL TASKS:

• Reviews pre-charts for evaluations.

• Schedules and pre-transplant evaluations and obtains results for coordinator review.

• Assists with clinic triage.

• Schedules any other testing as required by coordinator. Obtains results for coordinator review. Files results in a timely manner.

• Tracks and schedules follow-up procedures and appointments for post transplant patients.

• Organizes and maintains patient charts, filing, obtaining correspondence and test results, and sorting results which need physician review prior to filing.

• Documents in the transplant database any phone encounters (phone triages) with pre/post transplant patients.

• Assists transplant coordinators with patient presentation documentation in Transplant Committee to assure accurate patient follow-up.

• Charts clinic lab data onto flow sheets for clinics.

• Removes calls from voicemail as directed by the coordinators. Returns calls as instructed by the coordinators (including prescription refills).

• Prepares for clinics. Including obtaining results, updating flow sheets (including charting discharge summary labs/outside labs & outside clinic notes).

• Assists with clinic preparation as needed, including preparing lab requests, stocking clinic cart.

• Proofread and corrects transplant clinic notes.

• Assists data entry operator’s with questions regarding clinical information for data entry.

• Adheres to internal controls and reporting structure.

• Performs related duties as required.

REQUIRED EDUCATION / EXPERIENCE:

• Associate’s degree or equivalent and two years of related experience.
