Job Description

American Traveler is seeking an experienced Physical Therapist for a home health assignment requiring at least 2 years of experience and a valid CA PT license.

Responsibilities

• Position is in a home health, post-acute care setting
• Work schedule is Monday through Friday, 8:30 AM to 5:00 PM, with rotating weekend coverage (typically 1 weekend every 5 weeks)
• Each shift is 8 hours in length
• Mileage is reimbursed per GSA guidelines
• Uses Netsmart EMR, with HomeCare Home Base (HCHB) experience preferred
• Main responsibilities include assessing, evaluating, and providing physical therapy services according to a physician's plan of care
• Involves evaluating home environments, instructing patients and families on therapeutic appliances, and participating in care planning
• May train patients in the use of prosthetic devices and supervise Home Health Aides involved in rehabilitation
• Monthly team meetings are required for mileage timesheet verification
• Two professional references required: one supervisor reference from within the past 12 months and a peer or supervisor reference from within the past 3 years
• Candidates may not have been directly employed by CommonSpirit, CHI, or Dignity within the last year, but PRN through outside firms will be considered
• Requires participation in ongoing staff competency assessments and compliance with relevant Medicare, OSHA, and licensure regulations

Requirements

• Active California Physical Therapist license required; license required if applicable
• Minimum 2 years of recent experience in a healthcare setting, with home health and OASIS experience required
• Current BLS certification and NPI number required
• Valid driver's license and proof of automobile insurance required
• Strong oral and written communication and organizational skills required
