Position Summary:

VitalHire is seeking a skilled and compassionate PT - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Rye, NH. Key Responsibilities:

Provide direct physical therapy services to patients in skilled nursing facility setting
Develop and implement individualized treatment plans based on patient assessments
Conduct initial evaluations and ongoing assessments of patient progress
Document patient care and progress in accordance with facility and regulatory requirements
Collaborate with interdisciplinary team members including physicians, nurses, and other therapists
Educate patients and families on treatment goals and home exercise programs
Maintain accurate and timely documentation of all patient interactions
Participate in care plan meetings and discharge planning
Ensure compliance with state and federal regulations governing skilled nursing facilities
Maintain current state physical therapy license and required continuing education
Adhere to facility policies and procedures
Work scheduled shift hours from 08:00 to 16:30

Job Requirements:

Active, unencumbered license in NH

Minimum of 1-7 years of relevant experience, depending on specialty

Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements

Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

Current Resume listing detailed work history (MM/YYYY format)

Two current clinical references

State-required professional license

Physicial exam (completed within the past 12 months)

TB skin test or Quantiferon Gold (within the past 12 months)

Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)

Current Tdap and Flu vaccinations

Must pass background check and drug screening
