Job Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of recent skilled home health experience and OASIS proficiency.

Responsibilities

• Home health setting serving adult patients in the Anchorage area
• Assigned to 4-6 patient visits per day based on territory demands
• Productivity standard is 27-30 points per week
• No weekend or on-call coverage required
• Typically day shifts from 8:00 AM to 4:30 PM, five days per week
• Documentation utilizes Home Care Home Base and OASIS systems
• Patient care is typically 1:1 and travel between clients is required
• System efficiently groups patient visits to minimize windshield time
• Mileage reimbursement at IRS standard rates
• Free parking available at the office
• Primary responsibilities include home health evaluation, treatment planning, and care for adult patients
• Documentation tools will be provided and must be returned after assignment
• Any color scrubs are acceptable for daily attire
• Travel pairs accepted but each traveler must have their own transportation and will be assigned their own territory
• First-time travelers will be considered for this role
• Requested time off must be submitted with application
• Former permanent employees must have a minimum 6-month separation period and HR approval to return as travelers

Requirements

• Active AK PT license required if applicable
• Minimum 1 year of recent skilled home health physical therapy experience within the last 3 years
• OASIS documentation experience required for Start of Care visits
• Current BCLS certification required
• Experience with Home Care Home Base (HCHB) is preferred
• Valid driver's license and personal transportation required
• Ability to communicate and document in a timely manner
• Completion of Hospice/Home Health checklist required for consideration
• Must be able to travel within a territory and occasionally up to 100 miles in radius
